RIDER COMPLAINT FORM

 

This form is for non-emergency communications only. To report an emergency, please dial 911 (where applicable) or local emergency first responders.

Riders may use this form to file complaints about Pine Bluff Transit service. Complaints must be submitted within 10 days of the occurrence or incident. Include your phone number and/or email address so we may contact you. If you have not received a response within 10 business days, phone our office at (870) 543-5130.

Within ten (10) business days of receiving a complaint, Pine Bluff Transit will review the submitted information and send the complainant an acknowledgment (letter or email) informing them whether or not the complaint will be investigated further.

All valid complaints of programs, services, and activities will be investigated within 60 days of receipt of the complete complaint. Pine Bluff Transit may contact the complainant if more information is needed to resolve the complaint.

The complainant will then have ten (10) business days from the date of contact to submit the additional information. Pine Bluff Transit may choose to close the complaint if the requested information is not provided within the ten (10) business days. Likewise, Pine Bluff Transit may close the complaint if a complainant no longer wishes to pursue the matter, or if the complainant fails to cooperate in the investigation.
